To register, parents must have their child's official birth certificate and provide proof of where they live. Acceptable documents for showing proof of residence include a lease or rental agreement, current utility bill in their name, or a mortgage agreement. A driver's license cannot be accepted as proof of residence. School officials will make copies of the documents and return the originals. In addition to the required documents listed above, parents are encouraged to bring their child's vaccination records and Kindergarten Physical form to registration.
State law requires that all children entering school have evidence that they have received certain required vaccines: four polio, five (5) DPT/DTaP shots, three or four Hib (Haemophilus influenza type B), two MMR (Measles, Mumps, Rubella) doses, three Hepatitis B, and two Varicella (Chicken Pox). Children also must receive an official Kindergarten Physical.
Within 30 days of starting school in August, families will need to provide documentation of their child’s Kindergarten physical (must be on blue state K health form) as well as a copy of their child's official vaccination records.
If parents have questions, they should call the school where their child will attend kindergarten (see contact information above).
